Vice President, Ombudsman Services

Rachel Tate, MSW, is Vice President, Ombudsman Services at WISE & Healthy Aging and oversees all aspects of its LTC Ombudsman Program in Los Angeles County - the largest county in the state by population - with seven offices covering more than 1,800 facilities and 76,000 beds in skilled nursing and residential care facilities.

Having joined WISE & Healthy Aging in 2007 as part of our Ombudsman Program, Rachel successfully advanced to positions with higher levels of responsibility over her 15 year tenure with the organization.

Most recently, Rachel served as Administrative Program Director of our City and County of Los Angeles Long-Term Care Ombudsman Program where she oversaw our Pasadena and Palmdale/Lancaster Ombudsman offices. In this role, Rachel masterfully leveraged community partners to improve quality of care for long-term care residents. She also supervised special projects throughout Los Angeles county, led Ombudsman Program efforts to train local law enforcement, regularly trained 911 operators on how to manage elder abuse calls in both community and facility settings, led training efforts for fire departments on signs and symptoms of abuse in facility settings and how to engage the ombudsman and other community agencies to reduce the number of inappropriate 911 calls that come from skilled nursing facilities.

Rachel has been honored with several awards from prestigious organizations numerous years in a row, including the Glendale Fire Department, USC School of Social Work, Cal State Bakersfield, and the National Consumer Voice.

Rachel holds a bachelor’s degree in Social Science from San Diego State University. Her master’s degree in Social Work is from Cal State Bakersfield, and she is about to complete her licensure for LCSW designation.
